About the Role:

The Care Coordinator will play a key role in planning and managing care packages for service users across Almond Flex Limited’s supported living services. This includes liaising with clients, families, support staff, and external professionals to ensure the delivery of high-quality, person-centred care.

Key Responsibilities:

• Coordinate care packages tailored to individual needs

• Schedule and manage support worker rotas

• Conduct assessments, risk reviews, and care planning

• Liaise with local authorities, health professionals, and family members

• Monitor care delivery, documentation, and service user progress

Requirements:

• Previous experience in care coordination or a similar role

• Strong organisational and time management skills

• Knowledge of CQC regulations and safeguarding

• Excellent written and verbal communication

• Ability to adapt in a fast-paced care environment
